They dont actually. Real Madrid rarely plays with 4 midfielders. Even the current squad, they play with 3 midfielders i.e Kroos, Camavinga, Valverde. They play using 4-3-1-2 formation where Bellingham plays slightly behind Vinicius and Rodrygo. If Mbappe joins them they will have to change the formation to suit him.
