How does such a thing happen?
I always thought ship, especially destroyers and battle fleets have state of the art radar systems to detect objects coming near the ship some as little as a torpedo.

but from your questions of why the state if the art ships collide, i think its because such colossal ships take time and distance in order to change direction so it makes it very difficult to change course when another vessel is heading your way…
