Human error, mainly. People simply have a culture of driving carelessly, with a million distractions: driving too fast, and getting too close to other vehicles, changing lines abruptly without signaling, or signaling meaninglessly, loud music, talking on the phone, (even texting!), playing while at the wheel, eating/drinking while driving, driving drunk or otherwise intoxicated, or driving under extreme fatigue. All these are serious contributors making people to ignore basic precautions, which don’t really change, whatever kind of vehicle one is driving.
