Every month we examine all sentinel events (which are “never events” things that should never happen but did happen, unfortunately) as well as “near misses” which are events that could’ve had a bad outcome but we got lucky. We then analyse where the system let-down occurred and how we can prevent it from happening again. You really get to know the ins and outs of each department…maybe that’s something you can consider implementing.
