It’s not entirely accurate and he seems to be missing out some important things.
Firstly it’s not caused by hair falling into your pants, at least not always. It can just be an ingrowing buttock hair (natal cleft).
Then his “pressure” at the base of his spine is usually an abscess. Surgery initially will just be incision and drainage which might or might not include packing dressings for a while.
It is only later once the sinus has been identified that the more radical "laying open* will be performed.
