If you choose to report deaths with disregard to COVID-19 then you will have very few cases to report. Plus deaths occur everyday so it’s easy to conceal deaths that occur as a result of COVID-19 infection.
For example, Mkapa died of heart attack and as far as I know. This is one of the eventualities of COVID-19 and there is a likelihood that he died of the disease.
Tanzania accepted people will die but they won’t report it as COVID.

I had created several predictive models; a combination of both deterministic and stochastic models but going by current events, they were quite wrong. It’s a possibility. But in a country with no restrictions or interventions whatsoever (read Tanzania) mortality will be higher. But this can be masked/under-reported or hidden.
In Kenya interventions such as restricted transport (reduction of Road Traffic Accidents) improved hand hygiene (reduction in diarrhoea diseases) may reduce overall mortality. That is they will counteract the effect of increased COVID-19 deaths. In one day 40 people can die in a single road accident yet we haven’t reached 40 COVID-19 deaths per day so far.
Lastly COVID-19 disproportionately affects those with underlying conditions so what countries are experiencing is accelerated deaths. But deaths that might have occurred anyway. So it’s easy to call it diabetes, heart attack, cancer etc and withdraw the COVID-19 label

Death by definition (99% of the time) starts with cardiac arrest followed shortly by brain death. Whether it’s a car accident, drowning, gun shot, poisoning everything ends up in cardiac arrest then brain death (due to lack of oxygen). The only time you can have brain death preceding cardiac death is TBI (traumatic brain injury…like gun shot to the head, large brain bleed, massive stroke).
However, the way most people understand cardiac arrest is the understanding that the primary disease process originated from the heart. The most common, serious heart conditions which lead to death are coronary artery disease (blocked heart vessels) and rhythm disorders. If I could take a guess, Mkapa given his age, gender and body habitus most likely suffered a heart attack from an undiagnosed coronary artery blockage ->>> cardiac arrest.
